This contract is a binding agreement for service between the host/hostess and Sugar Woogars Kids Spa. Sugar Woogars Kids Spa Parties must receive a final number of party guests, including a complete list of all names, as well as confirmation of party package choice, including any add-ons, no later than 5 days before the event.
The above information is imperative in determining how many staff members will be required to cater to your event
. • Waiver / Release Form Our services are for entertainment purposes, and we will not use powered equipment, cuticle tools or harsh chemicals during our manicure or pedicure services. We will attempt to use products that are considered safe for children; however, we cannot guarantee that there will not be an allergic reaction to products being used for skin, hair, and nails. Unfortunately, skin damage or minor allergic reactions can result from these kinds of activities.
Sugar Woogars Kids Spa is not responsible for allergic reactions due to any services, costumes or consuming any food or beverage before, during or after the party. It is the responsibility of the guest’s or parent/guardian to inform Sugar Woogars Kids Spa staff of any dietary restrictions. Sugar Woogars Kids Spa Parties staff is trained professionals and will work to ensure that your event and guests are treated in the most respectable manner and a safe environment.
• I, the undersigned, parent /guardian, assume all risk and hazards incidental to participating in the activity and hereby waive, release, absolve, indemnify and agree to protect, defend and hold harmless Sugar Woogars Kids Spa, their owners, staff, and participants for any claim arising out of any injury to myself / child to the fullest extent allowed by law. I, the undersigned, host/hostess of the participant, am fully aware of the potential dangers and risk inherent in this activity, including physical injury, death or other consequences that may arise or result directly or indirectly from participation in all Spa activities. “If any provision of this clause is held unlawful, void, or for any reason unenforceable by a court of competent jurisdiction, then any such provision shall be deemed severable from this clause and shall not affect the validity and enforceability of the remaining provisions.” Your signature below acknowledges that you have read and agree to this Waiver / Release Form Harmless Clause.
• Sugar Woogars Kids Spa Standard Photo Release For valuable consideration received, I grant Sugar Woogars Kids Spa the absolute and irrevocable right and unrestricted permission concerning any photographs that has been taken or may be taken of me or my child in which I may be included with others, to use, reuse, publish, and republish the photographs in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, including the internet, and for any purpose whatsoever, specifically including illustration, promotion, art, editorial, advertising, and trade, without restriction as to alteration. I release and discharge from any and all claims and demands that may arise out of or in connection with the use of the photograph, including without limitation any and all claims for libel or violation of any right of publicity or privacy. This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, and assigns of the photographer, as well as the person(s) for whom the photographs were taken. I am a legally competent adult and have the right to contract in my name and that of my child. I have read this document and fully understand its contents. This release shall be binding upon me and my heirs, legal representatives, and assigns.
• Cancellation Sugar Woogars Kids Spa must be notified within 72 hours of cancellation to be eligible to reschedule your party. Rescheduled party dates will be based upon availability of the spa. All deposits are non-refundable. A 50% deposit is required at the time of booking. The remaining balance is due 1 week before the party.
• We will contact you a few days prior to your event date to confirm all of the party details. We look forward to hosting your Royal Celebration!
• The undersigned acknowledges that they are ultimately responsible for all services in accordance with our cancellation policy noted above.
